[Bug]: Stuck spinning/working when Supervised is selected

Steps to reproduce

1. start an app
2. add an empty directory as project
3. select CC model (I cant try codex atm) in UI
4. select Supervised in UI
5. tell it to create a simple task that modifies something... like: create a dummy empty file
6. spinning endlessly....

Expected behavior

It should ask me to give permission or just make a plan

Actual behavior

I made my own package for Nix converted from AppImage: https://github.com/matejc/helper_scripts/blob/master/nixes/t3code.nix (maybe missing something?)

I use cloud-code v2.1.81 (stable), installed separately

Might be related to: #1241 (but in that issue the UI still shows Full Access)

Workaround

I use Plan + Full Access for now

just noticed that going to settings and forwarding backwards fixes it temporarily. I used to reopen the app which takes a bit more time.
