Create new channel - Request fails with status code 500

Observed behavior

Each time I attempt to create a new channel I see 500 errors in the Console.

Expected behavior

There should be no errors in the console.

Steps to reproduce the issue

1. Sign in to studio and click the New channel button.
2. Observe the browser Console.
