This PR fixes the deprecated rate-limiter standard headers on patient portal routes and implements patient email verification, including generation, verification, and resend endpoints, a requirePatientEmailVerified gating middleware, unit/integration tests, and corresponding frontend verification screens in PatientLogin.tsx.
