- Add sign() method to TokenVerifier for creating HS256 JWTs
- Add TokenSignInput model for sign parameters
- Add headers.py with format_capabilities() and parse_capabilities()
- Add AGENTAUTH_HEADERS constants dict
- Add AgentAuth-Capabilities header to guard response headers
- Fix AgentAuth-Challenge-ID -> AgentAuth-Challenge-Id (match TS)
- Add tests for sign (3), headers (6), guard capabilities check (1)
