fix(tui): preserve directory context after moving a session

The /move command can relocate an existing session to another project directory without adding the directory-change reminder used by the previous workspace warp flow. This can leave the model operating with stale filesystem context.

Patch conflicts during a move also surface raw Git output without explaining that destination files may conflict.
