• A brief note on steps to reproduce (e.g., what you were doing when the historical tasks appeared) would help

I am working on a focused fix for this. The current direction is to clear stale session todos when a new turn is submitted so old todo dock items do not persist into the next request before fresh todo updates arrive. I have a small app-side patch and a regression test for the submit path.
