This PR fixes most JavaDoc errors discovered from RxJava 3 preview's javadoc generator (doclint enabled).
(I was unable to make gradle in RxJava 2 to report the same errors regardless of source level and underlying Java 8 runtime locally.)
Changes include:
<dt>in<dl>s<td>to<dt>in<dl>s>in<code>blocks with>(as it is forbidden in doclint of Java 8)<in<code>blocks with<initialSeedmentions fromscanoperators that take aseedSupplierand remove the deferred example as well<code><pre>to<pre><code>.
