issue template for revdep checks

we could make an issue template for posting revdep check failures that should be fixed before next CRAN release.

for data.table maintainers (not based on https://github.com/Rdatatable/data.table/wiki/revdep-issue-template which is for issues in other repos).

Hi @tdhock , I'd like to take a shot at this. I've prepared a YAML issue form template based on the "Steps to report a new revdep check problem" wiki section (not the revdep-issue-template wiki, which is for other repos). Will open a PR shortly.
